The Story Behind Revolutionary Care

As a primary care family physician, I have seen and encountered an ever changing medical field. Unfortunately, in the past couple of years, I have witnessed a vast problem emerging; the high cost and poor quality in the current healthcare delivery system are a direct result of the thoughts and attitudes of providers, patients, and society which have resulted in a system that is unsustainable. The increased burden on primary care provider teams makes it difficult to adopt evidence based processes and technologies that would promote access, quality and efficiency. The value of lean and quality improvement processes has not bee recognized or uniformly applied in the healthcare arena. The traditional medical model has shaped physician-centered attitudes that isolate physicians from shared responsibility with patients and team members. The current model does not provide reimbursement to providers adequately for spending necessary time with some patients. The current system does not facilitate full participation of qualified personnel because reimbursement is not provided when some team members work up to their full potential. It is with these discoveries that I decided to change my practice into a patient centered medical home, thus, transforming my own attitude and my team's attitude. This revolution in my life became my own professional and personal transformation.

After my entire facility was transformed, I began understanding the full concept, principles, and benefits behind a patient centered medical home. I then began a long journey in becoming a medical transformational facilitation expert so that I too could bring my knowledge to other providers in transforming their practices. Thus, the birth of Revolutionary Care.

At Revolutionary Care we gear you up for a transformational change. We motivate provider teams to examine and change attitudes. We identify, inspire and support practice champions. We train practice coaches to share with other practices.We access current practices to find places where burden can be lifted or reduced. We develop a plan to align gears of attitude and work flow. We educate about lean/QI and healthcare. We provide or connect with resources to bring about paradigm shifts and process adaptation.We assist in creating ‘barrier-free’ team-based models of problem solving that will support any type of change. We assist in development of a population management process to support the practice in any reimbursement system. We assist in the development of a care-coordination mind set and system to maximize available resources including current staff, community resources, professional organizational resources, and under or over utilized reimbursement/bonus monies.
